Experiencing an unforeseen life event is never easy, which is why we want to make this process as simple as possible.
 

Life Claims

For all Life claims, please contact your credit union to help you begin the process.

Before you call, you will need to have the following information ready:

  • Full name of insured person
  • Certificate number or enrollment number
  • Loan account number
  • Funeral Director’s Death Certificate or a Provincial Death Certificate
 

Critical Illness, Disability and Loss of Employment Claims

If you need to file a claim, here’s how:

  1. Call CUMIS Life Insurance Company (CUMIS Life) at 1-800-263-9120 to speak with a Client Service Representative to initiate the claim process.
  2. Within 48 hours you will receive a call back from a Claims Examiner who will gather all the relevant details regarding your claim. The claims Examiner will advise you of what to expect next and answer any questions.

What to Expect When you Call CUMIS Life

Critical Illness and Disability Claims

The initial call will take approximately 20 to 25 minutes to complete.

Before you contact us, please review the Critical Illness and Disability Claims checklist below and ensure that you have the requested information available prior to calling:

  • Name and address of your physician(s)
  • Name and address of your employer
  • Details of Treatment – i.e. Medications and dosages
  • Date last physically at work
  • Date of diagnosis (required for Critical Illness)

Loss of Employment Claims

The initial call will take approximately 10 minutes to complete.

Before you contact us please review the Loss of Employment claims checklist and ensure that you have the requested information available prior to calling.

  • Date last physically at work
  • Name and address of your employer
  • A copy of your record of employment
  • Copy of Employment Insurance (EI) approval
  • Copies of EI statements or copies of bank statements showing EI deposits or copies of Service Canada electronic confirmations
  • Details of any severance package received

Need more information?

  • Refer to your certificate of insurance for information about eligibility, benefits, exclusions, limitations and termination of benefits
  • You can obtain information regarding the status of your claim by calling our Claims Centre from 8 a.m. to 8 p.m. Eastern Standard Time
  • Your credit union or credit card provider can confirm if a benefit payment has been received

Privacy

Protecting your privacy is important to us. Privacy legislation requires that we do not disclose information relating to a claim to anyone but the claimant or in the case of a Life Claim, the estate representative. When you are calling for the status of a claim we will verify key pieces of information with you to ensure you are the claimant or estate representative.
 
If the degree of your disability necessitates that we discuss your claim with someone other than yourself (i.e. your spouse), we require written authorization from you to do so. If we do not have your written authorization to speak with the individual calling we will ask if you are available to provide verbal authorization for the call. For Life Claims, we will confirm we are speaking with the estate representative.
