Critical Illness Claims

In addition to the general information, the following is needed for a Critical Illness Claim.

(Please note: Critical Illness claims can be reported to us by phone or you can notify your credit
union directly. Conditions specifically covered under this coverage are outlined in your
Certificate of Insurance.)

• Completion of our Credit Life Insurance Claim Form
• Completion of our Claimant Questionnaire
• Copies of your loan documents and statements six months prior to
  date of diagnosis
