you contact us, please take a moment to read the important instructions below. If you need to
contact us by fax, mail, or email, the contact information can be found at the end of these
instructions. For your convenience, we have included a Claim Record which can be printed to record details concerning your claim.

During the initial call, we will ask you for important 1) general and 2) claim specific information. We ask that you make this call yourself and have your information ready to ensure your claim can be processed as efficiently as possible.
